If you need to freeze or request to cancel your membership, please do the following:

If you are requesting to freeze multiple memberships in family unit, please complete a separate form for each family member.

You can complete and submit your Freeze or Cancellation request directly on the website. If you are within your termed membership agreement, please also upload verification of relocation or verification of disability, directly to the online form. Your completed form is then automatically submitted to our Member Services team to be processed. Please keep in mind that we require 15 days notice for all freeze and cancellation requests. You will receive an email confirmation & PDF copy of your Freeze/Cancellation form.
